Application Procedures

Bachelor in Health Sciences leading to a Professional Master of Science in Occupational Therapy

  • Applicants must complete the application for admission to Florida International University which can be obtained from the Office of Admissions, Florida International University, University Park Campus PC 140; Miami, Florida 33199.

    A copy of the FIU application must be included in your OT application packet.
     

  • Applicants must also complete an occupational therapy department application and mail it directly to the OT department.  Applications will be accepted beginning in January until Feb 15th.
     
  • Applicants must submit official transcripts to FIU with their application.
    One transcript from each College/University attended must be sent, to the ADMISSIONS office.
    COPIES of all transcripts must be sent to the occupational therapy department even if the student is a current FIU student. Transcripts must include grades for courses taken during the most recent semester preceding the application.

  • Send all required documents
    Copy of FIU application-if not currently enrolled at FIU; OT application and all transcripts to date) together in one packet. Packets must be mailed Certified/return requested. The certification receipt will serve as verification that your packet was received. PLEASE BE ADVISED THAT WE ARE NOT RESPONSIBLE FOR ANY MISSING DOCUMENTS. Please do not attempt to verify by phone.
     
  • We must have transcripts of all course work through the Fall semester (December). Only transcripts of grades earned after the original application is received will be accepted in separate mailings. IF EITHER FIU OR THE OCCUPATIONAL THERAPY DEPARTMENT APPLICATION OR ANY TRANSCRIPTS ARE OMITTED, YOUR APPLICATION WILL NOT BE CONSIDERED.

Professional Master Application Procedures

Applicants must send separate application packets to each of the following two departments (total of 2 packets) by April 15th for fall term admission. Review of qualified applicants will begin in January for fall term admission. Late applications will be reviewed on a space available basis after April 15th.

  • OT departmental application (.doc format)
  • Copy of FIU graduate school application
  • Copies of all transcripts through the current semester. Only transcripts of grades earned after the original application is received will be accepted under separate mailing.
  • Resume
  • Statement of professional and educational goals
  • Recommendation letters
  • Copy of TOEFL (if applicable)
  • Copy of GRE scores (if applicable)

*The certified mail receipt will serve as verification that
your application packet was received. Priority is given
to applicants who have ALL THEIR MATERIALS
submitted by the April 15th deadline. After that date,
qualified applicants are reviewed on a rolling basis until admissions are closed.

Graduate Certificate in Gerontology

Candidates must be either a Registered Occupational Therapist, a student admitted to one of the MS OT programs or a student admitted to the Bachelor of Science OT Track program.
